If you are ordering your own birth record, you only NEED to take your ID. Knowing your SSN is helpful for the security quiz on the kiosks, but not a deal breaker. You would only need to take extra documents if you've had legal name changes. A driver's license and social security number. You need to prove who you are to get it. You can get a birth certificate for your self or your child only.
