If it was a 10 year passport you will need to start over. They only take passport renewals less than 15 years old. For a new passport you will need: proof of U.S. citizenship (Original birth certificate), Goverment issued photo ID, Color Passport photo (Most Costco's do them), DS-11 application form(Can print off internet), and a check or money order for payment. If you do it by mail you will get your original birth certificate back with your new passport.
