I filled out the online CLIENT INTAKE FORM, and got a call for a phone appointment 2 days later. And the advice was very refreshing, he gave me enough information to do the research myself but the confidence to know my rights. It had to do with APT evictions, I was so impressed with their help, I donated to their cause. Remember its free, if you want better service, then you should hire a lawyer.

Yes King County Bar Association should be able to provide those forms if not you can go to the courthouse in your county and should be able to get them.

I am treatment and have a specific schedule to follow,but this allows me not to miss time at work. By law do I have to provide a medical note from my doctor releasing me to work?
