Yes. You can get a birth certificate at any Ohio health department as long as you were born in Ohio. You can go to the website and order it online or just come in.

You can ask for one, but you get all the information and proof of vaccination in an email.

We have a fee structure for shots and take all types of insurance. However, no child is denied shots based on inability to pay!
