I needed certified copies of a couple of documents and put it off for a while because I was dreading going to the Vital Records office based on some of the reviews. Wow! I was so pleasantly surprised!! From the moment I walked in to their office till the minutes I left I had a great experience. When I arrived the security officer greeted me and was super helpful. There were two people in line in front of me and two windows were open with customers at each window. I was expecting a long wait but it took only 3 or 4 minutes. The lady in the middle window was awesome. I'm sorry I didn't get her name but I was there on Monday, October 3rd at around 10 AM. Anyway, once I explained what I needed she checked my paperwork and asked me to take a seat and wait till she called me back up to the window. Again I was expecting a long wait but she was lightening fast. It couldn't have been more than another 3 or 4 minutes and my documents were ready. The entire process took a total of maybe 10 to 15 minutes and I was out the door and on my way. A couple of things I want to mention... Because I'm not a fan of driving downtown now that there's a gazillion people living in Nashville, I parked at Publix on 8th Avenue and took an Uber to Vital Records. Second, I downloaded the forms I needed and filled them out before I arrived and I think that helped. And again I'm sorry for not getting the name of the person who helped me but if you have to go to Vital Records, try for the lady in the middle window. She's awesome!

They never answer the phone. I live in Memphis and now Im going to have to drive all the way to Nashville to figure out why we haven't received a corrected birth certificate by mail. I am very upset with this. Then again, Tennessee as a whole is just a trashy place

We need to get a copy of a birth certificate who was born in Altamont, TN. on July 20 1972 and we would like to come to the office @ 710 James Robertson Pkwy and pick up as we need this pretty quickly. Please advise if this is possible to do in 1 day and how much the cost is and what documentation will be needed.
A copy of your driver license or Picture ID a money order payable to the state you were born then need you fill out a birth certificate form that way they will know which state to send your money order copy of your driver license too

Go to their website and either fill out the proper form and send it through the mail with the fee or go to their office on James Robertson Pkwy. If you go to the office you will get it that day. I'm not sure how long it takes by mail.

I am sorry for your loss. I went to 710 James Robertson Parkway on 7/24/20 to try to get a death certificate, but they sent me to the Lentz Public Health Center (2500 Charlotte Avenue Nashville, TN 37209 615-340-5616) for the death certificate. There was a $15 fee and it took about 15 minutes. Best wishes.
You would have to call and see if they attached a tracking number but I think it may take a week or two depending on where you live.

Take a copy of birth certificate, marriage certificate and divorce decree to your local court registrar, complete request for name change (there is probably a service fee). Once local registrar has completed the change, get a copy of that document and provide to health department, which will record the change and update your record.
