I had the unfortunate experience of being transfered from the hospital to Osborne. The front looks great and my daughter choose this place. In the back where patients are has black mold on the ceiling and walls and my room smelled like mold. I was left alone for over two hours when first admitted. I had to use use the bedpan as I was not able to get out of bed. After two hours of ringing the bell for help an aide came in and told me I would have to pee the bed because she was so busy she couldn't get to everyone!!! I had a bedsore and that was not helping healing. The food was horrid, fried lard they called food. I never received any medications. I called my daughter, told her the conditions I was experiencing and she took me home after two days of living the Osborne nightmare. PLEASE do not use this center! They are careless, cold and non-caring!!!! place for me.

This was a terrible experience. I ended up leaving. Part of recovery is being comfortable and this was far from comfortable. Food was not edible. Would not recommend.

My mother was admitted to this facility on 09/02 and she passed away on 09/12. During her stay my mother stated she did not get the immediate requests required for her to be comfortable such as a proper bed for her size, extra blankets, warm food and on one occasion she did not eat for an entire day due to scheduling issues with her PT and Dialysis because of the nursing staff not making proper follow up phone calls in advance. My mother was sent out to a location for her Dialysis and was not seen due to a cancelation that took place the same day that morning she was supposed to be scheduled for and she stated she waited hours before getting picked up and made it back to this facility after 11:30 that night. Another occasion she was scheduled for Dialysis and they left without properly checking to see if anything further was needed and the doctor advised they could not see her for now a 2nd time for her appointment since this facility who was to provide the lifter failed to leave her one. She was alone for hours until finally getting picked up. The lack of communication and care is atrocious and the nurse staff needs a refresher on common decency and patience and I would never recommend this facility to anyone. This facility deserves No Stars.

My sig other was placed their last night, visited today,in my 40 years of health care,this place is totally disgusting, staff not friendly,every room I looked in was disgusting and dirty ! Should be shut down immediately !!!

This "5 star facility" is by far that! Maybe the front of the building was remodeled. The physical building according to the state is up to date. However, the way they tend to their patients is absolutely horrible! Filing complaints with whoever doesn't help. My Mom was left in her mess over night also! At 5:30 am her roommate got up and stood by the door until someone came to help her. She said I don't need help. She (my Mom) needs help. My Mom trees buzzing the nurses all night. Waved at the nurses walking by to no avail.

It’s beem my experience that vital signs are the CNAs responsibility I work at Osborn and have always assisted my nurse with vital signs. When something is wrong with the patient that’s when the nurses focus their attention but otherwise I have always been trusted to take vital signs like I was trained to assist. It’s crazy you don’t know how to or that that’s your role in assisting a nurse. And yes we take care of beriayric patients but they deserve care just like skinny people so I’m glad you left.
