PLEASE BE AWARE: Your time could be wasted if you go to this clinic. This place is very unprofessional. They say you need an appointment to be seen, but they never answer the phone. Once you finally go in to schedule an appointment, they have to decide whether they’ll see you or not!!! In my opinion Michelle Runnfeldt should not be running this clinic if she doesn’t like her position. And please don’t waste your time trying to report to the Big Boss (Laura Varnier) she will not return your calls.

This place has HORRIBLE customer service you will literally sit in the waiting area 2-3 hours after your scheduled appointment. The nurses and desk attendants are more worried about their LUNCH than your health. Don’t go there!

I have bad teeth and they hurt really bad
There's a Hope Clinic For Women downtown that does std and other feminine needs. Sliding scale. Planned Parenthood will do std tests for a cost. I don't know exactly what you're needing. Hope Clinic for Women also have psychiatrists. Sliding scale also. I don't believe Lentz has the equipment nor labs for the tests needed. I could be wrong.

Full name, date of birth, where she was born at, your identification and the fee for the printed copy. You will need to know if you want the Mother's copy or the Father's copy or even a certified original copy. Costs depends on what you need as well as where the child was born. Out of state births are referred to the state of birth
If you bring proof of income and residency, it is likely to be free or at a lower pay scale, depending upon your income
