The first time I visit this emergency room, I arrived at 6:45 p.m. and I had to separate about 3 hours to be able to be called and when I finally think that I will be attended, they left me sitting 30 more minutes on a stretcher waiting. I left at 11 p.m. of this place something never seen. And the saddest thing of all is that the children who go to this emergency have to wait just like an adult because they don't have an emergency just for children. There is very little medical staff for the number of people who come here. Maybe today was a day gone by but the wait was very long.

I called asking for medical records, they put me in hold for 20 minutes. I spoke with Ana and then she said I don't know what i am talking about. After she hangs up the phone. This is a horrible place to recommend clients. This is so ridiculous please don't refer anyone here. Ana is not a good employee please trust me.

I admired Filipina RN Sherlyn Valiente, she’s really a hardworking, comforting and reliable. She handled our discharge form accurately. She’s really the best. Hats off to you RN Sherlyn Valiente. Goodluck and always keep your smiling face.

I visited the ER at this location tonight. The medical staff here was amazing. I must admit, I was impressed with the the care, and the quickness of how things went. Maybe it was also a slow night. However at the end of my visit while waiting for my discharge papers, I was approached by a female name Shackile. She stated to me that she was here to collect a $100 co-pay. Her tone was unfriendly, and this conversation was happening while I'm sitting in an ER that's full of strangers. I did not feel comfortable of providing them with any personal bank information, so I told her that please bill me. She then began to talk to me in a condescending manner, about in the future this hospital requires all Co-pays are required to be paid at the time of visit. Meanwhile you have non tax payers on the floor smelling the place. The medical staff will be the only reason I visit this place again if I ever have an emergency.

Contact the hospital you want to take him to. They will put the rest in motion.
I dont have children. So i wont be able to help you. The emergency room is terrible the worst, no. O oclient privacy. HIPPA VIOLATION.
I highly recomend this hospital for any age very polite, the staff is so friendly and A+’s for every question but you may not like the answer to your question. Medical records is located a few minutes away from the hospital, however they are just as helpful as the hospital staff (Someone very high up was smart and made sure that all and I mean from Doctors to security staff to the cleaning crew were all drilled with the policy that a patient was to be treated with respect even if said patient is being rude I couldn’t believe my eyes when the nurse who I learned was finishing her shift could'veeasily sent him over to the new nurse sat there and listened to him until he felt “vindicated “. So I would go to another state To get my medical forms I would

Go to the Registrar of births, marriages and deaths of your state. They might be able to help by providing your details as well as your parents.
There is a parking lot next to the hospital on 66th. Pricey though. https://parking.com/nyc/lot/10207-66th-road Or you might get lucky and find street parking.

Stay far away from this hospital. I have stories to tell that would make your head spin.
This hopitals record keeping leaves a lot to be desired. Good luck in your endeavors. Best bet is to go there in person, you probably have to sign releases and pay for the records.
I know they have MBBS Students
I think you can complain to the social workers. Who are there for the patients rights amongst other things.
