If you were already seen by a GHD provider you would just call our call center to privide them with your approval number. It normally takes 1-5 days from there. She would likely qualify, you would need to bring her birth certificate, your ID, her medical documentation, the completed and notarized Patient Minor Form from the MMCC website, a note from her family doctor approving her for medical marijuana and the evaluation fee, card fee to get her certified.
