This is where your T-Mobile trade INS go. I believe they are stealing phones because apparently my trade in was never received. If you look on reddit there are many people going through the same thing with packages going to this location.

This is pretty much the biggest phone insurance/repair scam in the USA. Every phone that I have had replaced by them has had nothing but problems. The last one was a note 8 that I had a screen crack and was replace by a "refurbished" one. 3 months later, the screen went black. I sent it to Samsung for $500 worth of repairs and it has been working 100% ever since. But Assurant don't care about their poorly barely fixed phones. They just need the phones to work just long enough for you to have to pay that deductible again when the phone starts to acts funny again in a few months. The camera module on his Assurant repaired Note 9 died 2 months after. So if you ever have a phone replace by them, if it's not new, get ready to experience problems.
