Interesting displays about the Church of Jesus Christ of Latter-day Saints including some original artifacts in protective cases and a 180 degree movie. Very friendly volunteers. Some interactive experiences for young kids about temples. Currently has a display of art from winners of a recent competition. Also a souvenir shop. Worth visiting.

I was very impressed with the Church History Museum. I am learning the history of the Mormon faith and the museum really is Helping put the pieces together. I was fortunate enough to have a team of Missionary Sisters guide me through the multistory display. But the highlight of my tour was the art display put together by gathering youth artworks from all over the world 🌎. Very good and creative. Don’t miss it. One final piece of advice, it is good to have a Missionary team with you rather than being alone to insist on a picture of you giving an amazing talk at the conference 😃

You don't need an appointment. But I would check to see what time they open and close so you give yourself enough time to wander through the exhibits. You could easily spend 3+ hours there.
City Creek parking is a common parking spot.
I believe Portuguese speaking guides are available. There are Portuguese speaking missionaries available at Temple Square.
No the museum is closed on Sundays. I think next month they are changing the days and times they are open.

I imagine it's to give all the service missionaries and staff the day to go to church and honor the sabbath. The Church History Library, Family History Library, FamilySearch Center, Church Office Building, Church History Museum and the Joseph Smith Memorial Building are all closed on Sunday.
Yes, the death masks are real and the clothes of Hyrum Smith are the real deal too.
