I am not recommend this place. Doctor is ok there but service is extremely worse, visit few times, no one follow up next appointment, done the doctor visit totally no one follow up and what i need to do next no one tell, every thing just like self service. We are not doctor, we are patients, need someone tell and what to do next. i done a lab on June and try to contact them if i can see my lab report they said can't, have to wait for the doctor tell, but they never have any one tell me that i need to visit a doctor next. Today, they make a phone call said make an appointment the doctor will call me at 2:30 to tell me the result. i wait, wait, wait... whole day no one call me and got a message at the end said i was no show. really need to change doctor office. They are not care about patients at all...

I'm a patient here. As a Healthcare professional for over 30 yrs, and I never tell anyone unless necessary, good points and bad. I really never am able to get answers over the phone for some reason, and have been placed on hold for large chunks of time, or no one comes back to answer, especially towards the end of the shift. Everyone seems pleasant and kinda helpful, but the wait is horrible, on phone and in person. For some reason, the pre registration on line....well, the site drops every time. All in all, it's a different experience everytime, and just like anywhere else, different experience for every individual I'm sure.

This place cannot be called a primary care clinic, you cannot even say that they treat you, coming here is wasting time, here the only practice they use is disrespect for the patient, for me it should be closed

Our behavioral health services are integrated with our primary care services, which means you would first be scheduled to see a primary care provider to rule out any physical issues related to the symptoms you are experiencing. If they determine the need, they will refer you to our behavioral health team, which includes a Licensed Clinical Social Worker and Psychologists. They will conduct an assessment and offer therapy. If a psychiatric consultation is indicated, they will make one. We do not, however, start with psychiatry and medications--our approach is to work through other options. FoundCare uses a sliding fee scale based on household size and income. When you schedule your first appointment by phone (561-432-5849) you will be instructed on the documents to bring with you to verify income. During your first appointment, we will conduct a financial assessment, which will let you know where you fall on the sliding fee scale. Patients who are living at or below 200% of the federal poverty level pay anywhere from 75% to 25% of our usual charges. Patients who are at or below 100% of the federal poverty level are charged a nominal fee of $25 to see a primary care provider and $50 for a dental visit. If you have questions, don't hesitate to call!

The best option is to call 561-432-5849; often they can schedule a same day or next day appointment. If they tell you an appointment is not available, you can walk-in. However, expect to wait to be seen. FoundCare has a behavioral health program that is integrated with its primary medical care. We often find that anxiety/depression can be related to physical ailments. As such, a patient will be seen by our health care provider first. They will determine if there is a need to refer to the behavioral health program. If yes, then an assessment is conducted by a Licensed Psychologist or Licensed Clinical Social Worker and they formulate a treatment plan, which may or may not include psychiatry, as clinically indicated. For dental, we do basic dentistry--exams, fillings, exctractions, dentures, partials. We do NOT do crowns, caps, bridges, implants, or oral surgery. During the initial dental appointment, we do a thorough exam including x-rays and a treatment plan. You will be charged for this diagnostic visit. Based on the treatment plan, our dentists will perform the work we can do here at FoundCare and then refer you to another provider for any work that we do not perform.
