I got a replacement card a month ago, at that time they were not doing appointments for replacements. I had to print application, fill it out, attach my actual ID card and drop it off at the office. A guard allowed me in and directed me to the drop off box. No one else was in their office. They mailed my card back and 3 weeks later received my card by mail.

The office isn't open for in person services, they are working from home, call them and request that they email the application to you. You will have to print it off and mail it, and then provided everything is correct on your application you just have to wait for them to mail it to you. They do ask that you send the hard copy, of your license or id, whichever you have with your application.

I'd recommend calling them up to ask, if you can't find it in the web site. As I recall, what you need are the discharge papers from the hospital, your ID, and proof of residence. But don't take my word for it, it's been a while and those requirements change about every election cycle.
