I was referred to Nicholas after my ENT said there was nothing he could do about the ringing in my ear caused by ottosclerosis. Nicholas has decreased the ringing considerably and started me on a path of healing many other issues I have ignored for way to long!

Not sure what you mean by medical card. If you mean a health insurance card or health savings account card, no. One can pay out of pocket with cash, check, or regular credit card.
