What is happening with the nurses! I know Sage Creek is short staffing, but this is awful! They don't answer the call light! They don't answer the phone. When you do talk to them about your loved one, they say ok I'll tell the nurse, then you never see the nurse! This is rediculous! They are getting paid good money for my mom to be in there! I'm getting really upset! My mom calls all the time saying she hasn't seen anyone in hours!!!!!!

Sage Creek Rehabilitation Center is an excellent facility. They offer daily activities, a gym, dinning area, and cozy rooms (with a window you can open for fresh air, flat screen tv, AC/ Heaters, personal bathroom & showers). The physical therapists are amazing b/c they help motivate patients. I appreciate the equipment the facility has to lift the fragile elderly. The facility gives the elderly opportunities to socailize and a routine to help comfort seniors. The doctors & nurses are professional/ caring. Overall one of the better rehabs in the valley. My father-in-law Nick F. is currently a resident at Sage Creek Post Acute ; Nick is looking better & starting to improve by exercising his legs & trying to walk again.

The rehab place she was at wasn't helpful. She went there so they could help her to walk. She has been there about a week n a half n still not walking. She got worse, couldn't eat, stomach hurting n now she is back to square 1, liquid back in her lungs n don't know about her heart. (She developed “Thrush: fungal infection of the mouth” ) Her doctor gave medication once and never checked back to see if it worked and for how long! It came back but he didn’t as was told to us: her brother & sister in law on Sunday July 3.. I agree definitely that the nurses are slow because of understaffed or poor management. She is tired and frustrated. She wants someone to help her to walk. She is willing but she has to depend on PTs. Please continue to pray for her. That God will send caring doctors n nurses n PTs to help her through this ordeal. Please pray pray pray

Unfortunately, the mismanaged medication, faulty care and mishandling of a patient’s chart left her on an 11 day stay and a round trip back to the hospital. Blood levels out of whack, leaky legs swollen again, UTI infection never addressed and treated for dementia when it was the ignored UTI. No one to schedule follow up outside medical visits. Unless you will manage your loved ones care 2/7, I suggest to run away! Nobody is accountable, charts go missing and unread. Good luck getting a return phone call.

My mother in law was there right after release from St rose where she was treated for a stroke...after in home pt and ot she now is ready for outpatient...does sage creek offer outpatient services?

My loved one is in your facility. She is end stage cancer choosing ni further treatment. Is your facility equipped for comfort care or do we need to move her to another care program?

Is there a reason we aren't able to get through when we called. The phone rings and either no one answers or the receiver is picked up hung up to disconnect. Can we visit or find out the best time to call.

As far as I know the reception staff is not working at night. However the nursing staff takes calls when they are able.
