I believe it needs to be signed and have the seal. But this question is best answered by the City and Town Clerk. Call City Hall.
I don't know how much but I go to the clerk office first floor and they will give you all the answers
You can go in person and they will make you fill some documents with the informarion of the people that will get married. You could fill that up online on their website to save you sometime since they will already have it and will just look it up for your review. Both parties need to be there since they will check each persons ID and both need to sign the document. The fee is $50. Also if you know who will be performing the ceremony then have that information available. If not, I dont think it matters since you have 65 days to do the ceremony after you get the license. You download a request for a marriage certificate in the town that you are married in. So go to the southington town clerk website and see if you can request one to fill out.
I believe it's $20 for full size and $15 for wallet.
