There is a small form to fill out at the DMV.. They are accepting walk- ins without an appointment. Mondays and Fridays are usually Not good days to go. Don't forget to cancel insurance after turning the plate in. I Hope this helps.

If your from a different state you may need to make an appointment and go into the office. I would make an appointment regardless so that you can get your ID in hand on the same day.

I think you must first get one from your doctor to fill out and then bring it to the department of motor vehicle to process to give you a handicap placard
