You would probably have to reach out to the specific house that you were hoping to work with.

There is a credentialing unit in Albany NY 1475 Western Ave. Goggle OASAS WEB SITE OR CALL THEM.

If you contact OASAS or the website you can download that information. That's what i did. I receive the back round form and schedule my fingerprinting...and completed the finger printing.
