What kind of operation is this? I called and made an appointment apparently it takes over a month to get a basic vaccination...ok appointment made 35 days out THEN the next day someone calls me claiming to work for department of health and demanding to know my date of birth and what vaccination I wanted? Either your staff are tone deaf to identify theft concerns or your records are being accessed by actual crooks I'm not sure. If the goal was to make the patient nervous and convince them to pay more at CVS for halfway professional service I would say mission successful.

At the health department bring all shot records and ID for yourself and kids just in case

I went to Lee Health Department on the website
You can’t I’ve been all around town, was told the primary doctor first, then the site you were given the shot, was told they don’t make new cards unless u work in the lee memorial system, then I was directed to health department, no can due, I even wrote to the cdc and got email back sorry
I just went there yesterday. You have to have an appointment or call to make one. I called Thursday and they were nice to give me an appointment the next day.
