I could not have navigated what to do with placing my elderly friend and getting doctors on board and consulting with her. She is a wizard and if you ever have to place a loved one or need medical help for them, she connects you to all the right people. She is the best at what she does.

Tina and her staff provided services that took the burden off of us so we could spend our time and energy focused more on our loved one. Tina takes the time to actually listen to your suggestions and concerns and works diligently on your loved ones behalf. An advocate for your family. Losing a loved one is very hard, but Tina and Tara’s compassion for our family helped tremendously. I recommend them highly and infinite star rating. ❤️

I have collaborated with Visionary Care Consultants for close to four years. The care managers at Visionary Care have been extremely professional and handle the most challenging cases. Tina and Cynthia have been absolutely incredible in regards to crisis intervention, medication and symptom management. They help navigate the healthcare system while also ensuring their patient has quality of life. Thank you for all you have done to support our shared clients through the years.

We owe many thanks to Cynthia Smith who acted as an Aging Advocate for my elderly parents, approximately 10 years. She knew them when they were independent and became such an asset as their lifestyle began to change. We considered her "Boots on the ground" since none of the children lived in the state. Cynthia had resources and options available and convenient to my folks when it came time to transition from independent care to assisted and later skilled nursing. This can be so stressful for aging adults, the move, the change in activities, the mobility, etc. Cynthia was helpful when my mother was near the end of her life. She had resources to consider and choices for our family to make for her to spend her final days. With her professional background, Cynthia was aware my parents were missing proper mobility tools to make them more comfortable. My father was in a wheelchair that was far too small for him. Cynthia worked with me to coordinate between Dr., medical supply resource, and facility he resided to get him a comfortable wheelchair where he spent most of his last days. There are countless other examples of Cynthia's caring assistance in providing my parents a dignified plan for their final years. Thank you, Sally

Questions & Answers

Why choose an independent care manager OVER a care management lead home care team?

Many other companies offer care management services as a part of their portfolio of services, which includes in home caregiving. WE DON'T, AND HERE IS WHY. As you might imagine should there be a need for caregiving services, these companies will only present the option of working with their company. What if their company is doing a horrible job ? For over 15 years combined, we have prided ourselves on avoiding this potential conflict of interest, and continuously kept tabs on a plethora of caregiver agencies in our area. Because we are an independent care management company, we are confident that our recommendations will be just the right fit for your situation. We believe that is why so many physicians, trustees and elder law attorneys recommend our services. You will get an honest, unbiased assessment and a recommendation for continuing services you can trust.

For what type of clients are your programs NOT going to work?

Please know we are very selective in whom we work with, we choose carefully as we want to commit the most time we can to our clients. We do not take on more than we can handle because you and your loved one are the priority. We want to ensure positive results and if we feel that we are not a good fit we can refer you to another agency. This is not about us, this is about you having the peace of mind you deserve.

What type of personality do you work best with and what is expected of me?

The programs and packages were created for people who are absolutely excited and serious about getting the help they need it means not running around all day long, beating yourself up anymore saying: I dont understand the systems or I dont have any support They were created for you to put systems in place within a few months (sometimes weeks!), which will eventually lead you to peace of mind, ongoing support, and clarity regarding the resources available to you and your loved one. We are known to work best with people who are committed to success and who really want to change the lives of their loved ones for the better. Our clients often say just tell me what to do and Ill do it! You will have the support in place to achieve your goals and give you peace of mind.

Yes, there are government and community-based programs that provide some forms of support, however, these can be limited, fragmented and uncoordinated and many clients fall through the cracks. Investing in an independent Care Manager ensures that your loved one has personalized care coordination during every interaction.

Does my insurance cover these services?

Medical Insurance will often pay for short term interventions through Home Health Agencies. These agencies provide skilled nursing, physical therapy, occupational therapy, social services, speech therapy, and home health aides. Care management services are not currently covered by medical insurance, however, if you have a long term care policy, check your policy directly as some policies have coverage.

An Aging Life Care Manager can provide peace of mind for families who live far away. An Aging Life Care Manager is trained to identify problems as they arise and provide effective interventions. We act independently from any service agency so there are no overhead systems governing what can and can't be addressed. We can help preserve the independence and enhance the quality of life of your loved one. Having an Aging Life Care Manager on your side can reduce hospitalizations and save YOU AND YOUR LOVED ONE money in the long run.
