Super nice and professional customer service. Quick in and out getting new passports requests processed. Just need to have all paperwork, photos ready as per requirements posted on Consulate's website. Lady in the office was very knowledgeable and answered every question my wife and I had.

The consulate does not deal with evidence. They can only be made in Poland. The same applies to setting up an account at a Polish bank. If you have a passport and a Pesel, you can easily open an account. To obtain a passport, you will need a former passport (preferably) and / or a copy of a birth certificate, one passport photo. Hi Jerry, you need to make an appointment on the Consulate webpage to schedule an in-person visit. Download new passport request document from the webpage, and check what ather documents you need to bring with you, prepare also a photo for your new passport according to polish requirements.
You can book an appointment via the consulate's website and fill out an application on the spot. What you need to have with you is an outdated passport, a photo and a fee (PLN 50) in cash or by check. Currently, before entering, it is also obligatory to wear a mask.
Call the consulate. They will answer all your questions and point you to the right paperwork. They are very friendly and helpful.

My Daughyer is leaving in SeattleWa and her polish passport is not valid.Can she sent her polish passport to LA FOR RENEWAL? If not can she make appointment on line, since she bought already plane tickets for January 2022
You can download them from the Consulate's webpage but you need to print them on a A4 letter, if I recall correctly. Another option is to make an appointment and fill the forms on site.

Are you talking about a VAT refund? This all has to happen at the last airport that you are in when leaving the EU tax zone. They also issue you the VAT refund on the spot. One more note - for VAT refund you need a special form filled out with your passport info at the moment of purchase.

yes you are still Polish citizen, to give for your daughter Polish citizenship, she need to live in Poland at least 2 years, additionally you need to prove that you still have any connections to Poland (eg. you own something there, like house)
