I've never encountered this at Panera.
We've eaten at Panera over many years and as I recall our orders have been correct. Some times people have an off day or are training. Most businesses, if you graciously bring a problem to the clerk or managers attention they will offer to fix it or give you an adjustment.
I'm not there very often, but have found my order to be spot on each time. I'm sorry you have trouble with your orders.
