The only requirement they have is that whatever your kid is riding that it is age appropriate and they can properly operate it.

No just put his name on the waiver as a rider
no there isn't just need to be added on you waiver as a rider not passenger
I dont think so. Must wear a helmet though. Ita a bit rocky in many places but he should do alright.
