Give them your new address
You just give them your new address, they will either call or send a letter to verify from the new county. I did this between Lorain and Cuyahoga.
You shouldn't have to fill out another app. Just give them your new address which is a (change of address) you might have to fill a form out for that. Either on their website or go to the County office.
