I got my first vaccination on January 28, 2021 at the HCC Athletic Center. I am scheduled to get the second vaccination on Thursday, February 25th at 10:28 am. I don't see any form that I am supposed to print out with the time. Do I need one?

Hello. Thank you for the question. You will need to bring some form of photo ID and the vaccine card you were provided during your first vaccination.

You need to take with you photo identification such as a dri ers licence, and also the small paper form given you when you got your first first vaccination, called the "COVID-19 Vaccination Record Card." The 2nd vaccine team will probably be satisfied with just the card, though.
