Yes u need to have the disconnect notice, • Photo ID • Social Security cards for everyone in the household • Eviction notice and lease agreement • All utility bills • Receipts for rent Proof of income for all members of the household for the past month • DCF vision card if receving cash assistance or food stamps • Documentation of cause for need of assistance (medical bills, termination notice, auto repair invoice ect. On Mondays, call 316-267-0222 between9:00 a.m. – Noon and from 1:00 – 4:00 p.m.
If they haven't help you this year, I think they will, but you had to go to their place very early and be on line, you have to be there by 5am
