You have proof of the underlying cause of the chronic pain.. like I was hit by snowmobile as a teenager and had smashed leg that after 40 years causes me problems now. Pain has to have a cause, you show the cause of the pain
I was in an industrial accident in 2008 causing a lengthy surgery on my leg. Broke my tibial and had to have rods and screws in place. Does that qualify me for chronic pain? I have no insurance so I havent been able to go to the doctor for the pain. What do I have to do with no regular Dr.?
Call OMC, have them email you a release form, input all your doctors on that form, digitally sign the release and then submit. Schedule an appt. with OMC and go from there.

So you'd have to show a history that has been filed. My case is back pain I have delt with for the past 8-10 years maybe more. My doctor told me the pills prescribed would not totally kill the pain due to the fact I will need them the rest of my life and will increase in potancy in time, last visit was three years ago when I was sent to a rehabilitation center to stretch crack my back. I quit going to the doctors after that and started illegally medicating my self...quit my 20 year job to do so. And now I'm having s hard time here in ohio obtaining a card.
