I'm not a professional but I'm pretty sure you don't need your doctor to fill out paperwork you just need to get a copy of your medical records and book an appointment

^EVO is right. But spending about $400 for an appointment and the card I personally will hold off on until the program is running. Seems like a waste when realistically we won't have medicine until 2019 (try as hard as you want but you can't force nature, and these guys don't even know what they're doing.) If I get one now I'll be halfway to renewal and being out another$400 by the time it's ready.

Make an appt at a medical marajuana Dr office. I recommend the one in Westlake Ohio. It is $280. For the appt that covers all your appointments for 1 year. They do not take insurance. Take your medical records with you to appt. Also once you make your apot you will recieve an intake questionnaire by email to submit your medical information. After your appt you will recieve an email from Ohio pharmacy board to register for your medical card. You will oay a fee of $50. On their web site, and them you will be able to orint your card up. The Dr will give you a temoorary card in the form of a letter to use while you wait for the pharmacy board to register you. This letter can be used at the dispensaries, and as proof that you are a legal medical marajauna card holder. Good luck. It is really much easier than it sounds. (Sorry for the typeo's, my hands don't work so well. Lol) Also if you are on disability you can submit your proof of benefits letter and your fee is $25. 50% off

I am so confused by all this but taking 34 pills a day and still a lot of pain.,Do I need all the records pertaining to my conditions or just the diagnosis? If I call will they tell me when it will be available?

As Evo stated all you have to do is obtain a copy of your medical records (only info about your condition) and make an appointment. As Tom stated wait until the program is fully ready to give out cards. I have a $300 certificate and will have to repay when the actual card becomes available.
