Not sure what you're asking exactly. But they will ask you for an email address to which you will receive email confirmation and updates on the status of your card. I received an email 5 days ago notifying me that my application was received and approved. I just received my card in the mail today. :) Cheers!
We should have went over the application with you in office. An email must be provided by every patient. We also put our email down to track every application we process. Please call us to discuss your personal circumstance
