Not unless you need to prove citizenship. My advice bring everything they could possibly ask for and more. They denied mine because they couldn't understand dates on a military pay stub and I've used another name. Needless to say, I fixed everything in a second visit, but I live out of state and their hours aren't the best. The application doesn't cover every possible circumstance.
No, just Pa driver's license or Pa State ID. Carry additional information just in case utility bill that will prove proof of residency. (your passport doesn't hurt though) Check the Philly police website for further information.
