A replacement card/duplicate is $20. If you bring in all of the pieces of the card and surrender the card to the Food Handler Safety Training Card program, staff could do a courtesy reprint for you. If you do not have the entire card, you would need to pay the $20 replacement fee.
Usually duplicates are for those individuals that have lost their cards. Tape will do wonders, is still a valid card.
