It is not the centers job to help with a dog you adopted 5 years ago. It is apart of your family, you take it with you when you move and give it veterinary care.
The best thing to do would be to keep the dog. Taking the dog back to the rescue can cause intense trauma, and may cause her to get even more sick. Please find a good family member or friend to care for your pup if you cannot take them with you. Giving the dog to a shelter in no way guarantees it will have a great life, especially when it has lived in a home for 5 years.
If you adopted your dog from them and you now want to rehome it, take it back to them. When you adopted the dog you signed a contract stating you would give the dog back to them if something came back. They will want him back
