The Fresno Rescue mission is also an option. Also consider a treatment program. I wouldnt go it alone, there are people who care about you and can help.
I'm sorry bro I'm not sure about Catholic Charities on how much they do I only go there for food. But I wish you the best of luck prayers are with you.
