There actually are public restrooms, up the hill at the Blowing Rock City Park. Less than 3 minutes from the bathroom to the trailhead, past the upper side of the parking garage.
I have not seen any public restrooms anywhere nearby. I always go to a nearby gas station first. The trail goes steeply downhill, so rushing out to use a restroom would get you out of breath for sure.
