Yes, you will need to bring the full adoption decree with you. If more then one parent is listed on the court order, both will need to attend the appointment or prefill out a statement of consent to issuance of us passport. Please visit travel.state.gov to make an appointment and for further guidance.

Yes they can, both parents have to sign off on it unless there is a court order.
