Have anybody been ask for a marriage certificate or proof of dependents when u sign up for family gym after the contract is made and they never told u all that information before signing the contract???

Yes they asked me for all this information and I didn’t feel comfortable doing so

We had to have a Dr.'s note for proof of disability for our adult son and drivers license for myself and my spouse. Our adult daughter had to show her college ID which had to show our address for proof that she lived with us. They are pretty strict.

Yes. We also had to provide copies of our kids birth certificates. It is a very bad process. I did not have to provide this information when I purchased my house or my car.

Yes. For Senior Couple's membership, marriage certificate and driver's licenses were required. Makes sense.

We did have to provide some proof of relationship and dependents when we signed up our after for our 19 year old son who moved back home.

I dont recall ever being asked that at the rec center.
