If i am over 50, do i have to bring a note from doctor saying my health is ok to jump?

Hi Cynthia! There is no need for a doctor's note unless you have a condition that would normally disqualify you from being able to safely skydive but your doctor believes to be good enough for you to skydive safely.

You sign a bunch of waivers that state you are healthy to jump and you won't sue them if you die or are injured. No note needed!
