Yes you can have a carry on at no cost an one personal item
It really depends on what airline you are on and class of ticket you carry as to whtehr there is a cost, but so far all planes even with most basic fare allow one carry on minimum even if it's a cost. Usually one carry on and one personal item about the size of a purse.
