Unfortunately in the state of Florida there's a lot of Hoops to go through and everybody has a piece of the pie so what I did was I went on the website I can't remember what it's called and you start filling out the process then I did a online video with my laptop with a doctor that was $50 I think and then you were set up with a meeting with a doctor which was either $100 or $300 then you get your card after about 6 weeks and there's a couple more processes there I forget but be prepared to pay each step of the way it's not cheap. You can also do it over the internet to get set up. Then you must have at least one face to face meeting. I pay for the whole year. It's cheaper and I get a refill ever 70 days. When I needed to have an oral added and another increased by the amount I needed, it was a simple email and it was taken care of the next day. You do have to see a Doctor that are MMR certified and you must meet the requirements to revive a card. It's not cheap by any means which also depends on how much you need to help with your pain. You must visit a certfied MMR physician. Check the Internet or see if the dispensary has a list od Doctors.
