Have you purchased your PS4 within the last year and is your PS4 under warranty? If not, there are 2 ways to fix this problem. #1 is simple, #2 is a bit harder. #1: take a small needle or small pen and press the reset button on the back of the controller. This will reset the controller to factory and it will resync with the console, and if for some reason that doesn't resync with the console, take a USB micro cable and plug it into the first port of your PS4 and then plug it into your controller and that will also resync it to your console. #2: disassemble your PS4 completely. Disconnect and reconnect your Bluetooth module. Reassemble PS4. Holding the power button for more than 10 seconds you will hear a single beep then another single beep and then a double beep. Release the power button and it will take you into safe mode. Choose the option to restart PS4 normally. Let the PS4 restart normally and then when prompted press the PlayStation button on the controller and it will resync.
