I agree with Jennifer, you should visit your local Costco and fill out the form. Anything special, review with Bakery Mgs.

As far as I know you have to do this in the store.

I've never purchased a cake through Costco, so I cannot give you any factual information about that. My best advice is to call them.

You can call in to Costco and the prompts will give you instructions on how to reach the bakery

I know there is a form in the bakery area. I'm not sure it can be done from the website
