There is a local veterans out assistance office on Pittson Ave. In Scranton. 1002 Pittston Ave, Scranton, PA 18505 Also there should be a copy filed at you county courthouse house if that is accessible.
Visit your local VA office... if I am not mistaken you can request it on line also.
