Just go to that department of where you got your surgery and reguest a copy from thag surgery it's likely they will give it to you and you may have to sign a release form for your medical record.. Hopefully all goes well😊

Call first and ask if they go back that far

I am not sure. Because it's more than 10 years already. Anyway bring whatever record, ID you have and come and ask to Medical Record Office. Definitely they are here to help you. Good luck!
