I am requesting deceased Mother's birth. I filled out application online. It came back for signature. I can bring to notary and sign it. Do I need anything else in addition? Do I write anything in 15. under customer comments/additional information?

If you need the Birth Cert of your deceased Mom you would need to send with Application a Orig. Death Cert. if she did not pass within the 5 Boroughs of NYC or a Copy of Death Cert. If she did. We will then send you her Birth certificate. I hope this will explain things.

Did they also request the death certificate? They may ask for that as well.
