No appointment needed. Mondays and Fridays tend to be busiest. For faster service, try to be there when they open at 9am. Saturdays 10a-2p. Two passport size photos required, they offer to take those there also for an additional fee.
I wouldefinitly call first to see if they do internation licece NC E, but I know they don't do licenses for Ca. Only the DMV.
