I was reading that if there is two adults in the household that both adults have To be present...what if one is disabled... and can't get there ?

I am disabled and when my husband went he simply took my id with him. I didn't have to go with him.

As far as disabled persons go... you are also aklowed to pick up for the disabled that are not a part of your household by having them fill out an absentee form and getting their info recorded

Take both ids abd a piece of mail or a bill for proof of address. Then just your id after that except for 1st visit each July or your 1st time after that if you don't go in July you'll need bith ids and the proof of address again.

How do I get the form I need for my fiance to be able to pick up food for me or is it really as simple as letting him take my ID and bill?

You are able to include them at a produce market. If its at mid Ohio I know they have a proxy letter which ables you to pick up as well. You can always call and they can tell you what to do in this case to be sure. I hope that answer your question.
