Well now, that's quite a common practice unfortunately or fortunately between the patient and caregiver. The remuneration wasn't very well thought out. As long as there is mutual consent, there's nothing you can do. The burden of proof is not easy to distinguish.
You probably need to just accept it on an individual basis due to privacy policies, but contact your state reps. To recommend tighter controls on the program and ask if there is an inspector general or similar investigator you may discuss your concerns with.
