Follow the dmv site's recommendation and make sure you print out all the appropriate paperwork and fill it out, double check it and put it in an envelope thats clearly labelled. If you go to Enfield, the officers outside the door will direct to which box to put your paperwork in.
You need to bring all the necessary paperwork, probably doctor's note. And they should be able to transfer that, I don't know if they're doing it now because of the limited services they're offering.
