You can go to a DMV office or apply for a plate remake by mail. You will need to fill out two forms -- A registration application, found here: http://www.dmv.ri.gov/documents/forms/registration/TR-1.pdf And a plate remake form, found here: http://www.dmv.ri.gov/documents/forms/registration/RemakeLicensePlate.pdf If you want to mail in the forms, include the payment as well, to: Division of Motor Vehicles Attn: Plates, 600 New London Avenue, Cranston RI 02920.
