The SSA site does not have any instructions on where to send the W-4V form for Voluntary Withholding. The form says is to "give it to the payer, not the IRS". Do I mail it to the the Edsall Road location or do I need go to the SSA office?

Going to the location is the best option in your situation. But I will advise you to go there as early as the time it opens, if you dont want to be in a looong line...
